Your rights

you have the right to access your data, rectify it, erase it, restrict its processing, transfer it, object to processing, and lodge a complaint with the President of the Personal Data Protection Office (UODO).

Retention period

medical records are kept for 20 years. Monitoring recordings are kept for up to 3 months, and employee data for 10 years.

At our clinic, the wellbeing and safety of our youngest patients is a priority. We follow Minor Protection Standards:

• We always introduce ourselves to the child and explain the course of the visit in simple language suited to their age
• We respect the minor’s right to express opinions, concerns and to take part in decisions about themselves (from the age of 16, the patient has the right to co-decide on treatment)
• We are honest – if a procedure may involve discomfort, we warn about it in advance and ensure appropriate anaesthesia
• A parent or legal guardian has the right to accompany the child during the visit
• Staff are required to respond immediately if abuse of a minor is suspected

Every patient has the right to access their medical records concerning their health status and the services provided.

• Records may be made available for viewing on site, as a copy, an extract, a printout, or on an electronic medium
• The first copy of the records is issued free of charge
• We provide records to the patient, their legal representative, or a person authorised in writing

We use top-quality materials and proven procedures. We provide a 2-year guarantee on completed treatments, covering:

• Restorative treatment
• Fixed and removable prosthetic restorations (when replacing all missing teeth)
• Implants (surgical and prosthetic part)

Conditions for maintaining the guarantee: completion of the entire planned treatment and strict adherence to check-up visits and hygiene procedures every 6 months.

Exclusions: the guarantee does not cover damage resulting from the patient’s fault (poor hygiene, failure to follow recommendations, mechanical injuries, unauthorised repairs) or work for which the patient was informed that it is not covered by the guarantee. Complaints should be submitted in writing to the clinic’s address or by email.

We respect our patients’ time. We ask for your cooperation so that we can carry out treatment efficiently:

• Confirming visits: we confirm appointments one day in advance. Lack of contact and confirmation by 7:00 PM the day before means the appointment will be cancelled
• Lateness: arriving more than 15 minutes late is treated as cancelling the visit, and a patient in pain may be seen instead
• Cancelling visits and deposits: please cancel appointments at least 24 hours in advance. After two uncancelled appointments, a 50 PLN deposit is required before the next appointment. After three no-shows, we reserve the right to refuse further treatment (except in life-threatening situations)
• Payments: we accept cash, payment cards and BLIK. Bank transfer (prepayment) is only possible if the funds are credited the day before the visit

In accordance with the Atomic Law Act, we inform you that our use of X-ray equipment has no negative impact on human health or the environment.

• We do not release any radioactive substances into the environment
• Measurements and calculations of permanent shielding confirm that the permissible annual radiation doses (20 mSv for staff and 1 mSv for the general public) are not exceeded
